Fujisoft Health Insurance Association is working for members when they go to a medical institution due to sickness or injury and it seems that insured person paid most of the medical expenses required for treatment, or insured person were absent from work due to illness treatment or childbirth and not paid. We are engaged in various businesses such as injury and illness allowance, maternity allowance, lump-sum childbirth and childcare allowance, payment of benefits such as burial fee due to death, and various medical examinations and health guidance for prevention and early detection of illness.

Your personal information is indispensable for our union to carry out the above business and provide services to you, and it is the greatest to store and handle that information safely. Recognizing this as an issue, we will thoroughly implement it for all officers and employees involved in business activities and related parties.   2. The Health Insurance Association uses the personal information provided by members solely for purposes considered beneficial for members, such as health maintenance and promotion. In addition, it uses Individual Numbers only within the scope of the purposes specified in the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedure.
  3. Except when it has obtained advance consent from the member, the Health Insurance Association will not provide personal information of a member to any third party. Additionally, it will not provide personal information containing Individual Numbers (“identifying personal information” hereinafter), whether or not the individual has consented, except in the cases specified in the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedure. However, if personal information that is not specific personal information falls under each item of Article 27, Paragraph 1 of the "Act on the Protection of Personal Information" (May 30, 2003, Law No. 57), the subscriber's advance We may provide the personal information of the subscriber to a third party without obtaining the consent of.
  4. In addition, personal information that includes an individual number (hereinafter referred to as "specific personal information") will not be provided regardless of the consent of the individual, except as provided in the Number Act.

Regarding personal information that is not specific personal information, if the following items apply under the Personal Information Protection Law, the personal information of the subscriber can be provided to a third party without the prior consent of the subscriber. I will.

<Provision of Personal Information to Third Parties (Comprehensive Permission)>

14. Provision and creation of anonymized information

The Society continuously prepares anonymized information for use for health activities, epidemiological surveys, and other purposes, providing this information to the Rezept analysis vendor by electronic communication. The anonymized information prepared and provided includes gender, birth date, medical insurance eligibility information (e.g., period of membership), examination records from medical cost details, and health examination records. No information is included that could be used to identify specific individuals.
