When you lose your insurance card

First of all Note: Be sure to report the lost item to the nearest police station.
Documents to be submitted
  1. Report of loss of insured person's card (report of loss of health insurance card)
  2. Application for reissuance of insured person's card (notification of reissuance of health insurance card)
    • If you lose your health insurance card and want to reissue it, you need to report both 1 and 2.
    • Only 1 is required to return if you lost your insurance card when you retire the company.
Submission destination Submit to the Health Insurance Association via the health insurance office worker at your company.
