- Report of loss of insured person's card (report of loss of health insurance card)
- Application for reissuance of insured person's card (notification of reissuance of health insurance card)
- If you lose your health insurance card and want to reissue it, you need to report both 1 and 2.
- Only 1 is required to return if you lost your insurance card when you retire the company.
